The Eye Centre Hornsby

Place chart approximately ½ metre away with your reading glasses on

Cover one eye  

While looking at the central black spot

• Can you see all the corners?

• Do any lines look wavy or crooked?

• Are there patches where the grid is missing?

Cover the other eye,   repeat number 3.
